Learn what a GP for Me means to Hope community at Feb 24 forum

he Chilliwack Division of Family Practice is pleased to announce that the Partners in Health Care Community Forum will be taking place on February 24, 2015 from 6 p.m. 8 p.m. at the Hope and District Recreation Centre.


We will be discussing strategies being implemented in Chilliwack, Agassiz-Harrison and Hope as part of the province-wide attachment initiative, A GP for Me.


Working with doctors in Hope, Agassiz-Harrison, Seabird Island, and Chilliwack, we want more of our residents to receive the type of primary care they need. Attend the forum to learn how primary care is changing for the better in our community. A GP for Me strategies that will be discussed include:


PAM (Patient Attachment Mechanism):

Hope Clinic Expansions and Fraser Canyon Outreach
Physician Recruitment and Retention
Practice Support and Coaching
Community Education
